Née à Venise dans une famille juive (fille d'Amedeo Grassini et Emma Levi) ; elle épouse Cesare Sarfatti, avocat juif de Padoue, dont elle porte le nom. Critique d'art (Avanti!, puis animatrice du mouvement Novecento Italiano), elle fut la biographe et la maîtresse de Mussolini. Elle publia une vingtaine de livres. Bien que convertie au catholicisme, elle fuit l'Italie en 1938 après les lois raciales.
