Médecin, journaliste et dirigeant du mouvement ouvrier autrichien, né à Prague le 24 juin 1852, il abandonna sa pratique médicale à Vienne, où le contact avec la misère de ses patients l'avait conduit à étudier la question sociale, pour se consacrer entièrement au socialisme. Il unifia en 1890 les factions ouvrières autrichiennes en un parti fort, œuvrant pour le suffrage universel ; le 9 mars 1897, les ouvriers de Vienne votèrent pour la première fois et firent élire quatorze candidats, mais Adler fut lui-même battu dans sa circonscription par la coalition antisémite. Rédacteur en chef du « Wiener Arbeiter-Zeitung » depuis 1894, il traduisit aussi les « Paysans » de Stepniak.
